Everything you need to know before your first visit. We want your appointment to be as productive as possible.
What to Bring
We ask that patients come to their appointments prepared. Please bring the following, if applicable:
Required
- Photo ID (Driver’s license or government-issued ID)
- Insurance card (front & back)
- Imaging reports AND discs if available (MRI, X-ray related to pain)
- Referral (if required by insurance e.g. PPO)
- Imaging reports AND discs if available (MRI, X-ray related to pain)
Strongly Recommended
- Medication list (name, dose, frequency)
- Prior treatment records for this complaint (prior injections, PT notes, operative reports)
